Transitional living

We are committed to supporting our patients who have undergone a period of rehabilitation or following a crisis in the transition to their home environments. We have available an independent living flat with 2 bedrooms. Patients are supported by the care team (including their family or those close to them if they wish) while still having the healthcare professional “on-call”. and we have the staff to help our patients develop their individual skills to return home.
